Title: Wellness Nurse LPN/LVN Contract Location: Baytown, Texas Job Details: Pay Details - $15/hr $1125 weekly stipend (travel rate); $46.25/hour (local rate) Guaranteed Hours Shift - 7pm to 7am Hours- 36 Description of Job: The Wellness Nurse supports resident health and independence by coordinating care, liaising with families and healthcare providers, and managing wellness services in Assisted Living and Memory Care. This role supervises and trains staff, ensures compliance with care policies, and provides direct care as needed. Responsibilities: Ensure resident care aligns with Federal/State regulations and company policies. Supervise and train staff on medication delivery, emergency procedures, and documentation. Schedule staff coverage and coordinate tasks during absences. Conduct and update resident assessments, ensuring timely completion of service plans. Monitor resident health and communicate changes to relevant parties. Manage resident care data in the EMR system, ensuring accurate documentation. Coordinate medication orders and oversee storage/delivery. Act as a liaison between residents, families, and healthcare providers. Maintain safety standards and coordinate special provisions for residents with specific needs. Participate in government inspections and support compliance efforts. Manage department costs and supply inventory. Stay updated on dementia care practices and apply knowledge to improve resident outcomes. Provide direct resident care, including vital signs, ADL support, and health monitoring. Observe, report, and assist with care plan development within nursing regulations. Foster teamwork and uphold professional standards in interactions and appearance. Perform additional duties as assigned by the Wellness Director.
