Social Worker

A Faith-Based, Nonprofit, Short-Term Rehabilitation and Skilled Geriatric Care facility sponsored by the Carmelite Sisters for the Aged and Infirm, we've served the community since 1971. We are seeking a Social Worker - Discharge Planner.
$70K to $72K annually based on skills and experience.
Social Worker - Discharge Planner Qualifications:
Minimum of 2 years’ experience in a Skilled Nursing Facility (SNF).
Master’s in Social Work or related healthcare field.
MDS and Short-Term Discharge planning experience required.
Social Worker - Discharge Planner Introduction:
The Social Worker - Discharge Planner will collaborate with the Director to plan, organize, and oversee the Social Services Department's day-to-day operations, ensuring the highest level of care and support for our residents.
Social Worker - Discharge Planner Job Summary:
Plan discharge and referring residents to appropriate services.
Conduct interviews with residents and families to gather social histories.
Provide solutions for social and environmental challenges, including financial assistance, discharge planning, and referring to community agencies.
Complete all documentation in the electronic medical record - MDS.
Schedule care plan meetings.
Education of the care team and completion of advance directives.
Coordinating social service programs with administration, medical and nursing staff, and other departments.
Participate in state and federal inspections and surveys as needed.
